Patient Information
- Prior to Your First Visit: Make sure to have a copy of your doctor’s script (if you were given one), your ID card, and your insurance cards.
- Check In: Your care coordinator will review your insurance benefits or cost with you. Make sure to arrive 20 minutes early!
- Initial Evaluation: You’ll have a 1:1 full evaluation with a physical therapist. Be prepared to discuss your medical history, the reason for you attending PT, and your goals for physical therapy!
- Treatment Plan: Your PT will offer a treatment plan and home exercise plan based on their findings. You will work together to create meaningful goals for your time in PT.
- Visits Following Your Evaluation: We schedule follow-up visits after your evaluation. Your PT will provide a recommendation for frequency of attending. Be honest with your PT about what you’re able to attend!

We accept all insurances but are currently out of network with United HealthCare. This means you can still come to PT at Empower if you have UHC, it will just be billed toward your out-of-network benefits.
We also offer payment plan options and utilize Cherry to help patients pay for their PT services over time.
At Empower we also accept workcomp and auto accident cases.
If you have ANY questions about your insurance, payments or billing, just call any of our offices and they will walk you through your plan benefits and coverage!
We offer multiple ways to help you pay for your physical therapy services! The most popular option is via Cherry. Click the tab on our website header that says Pay Here -> Payment Plans to learn how you can pay for your PT services over time using Cherry!
We also provide discounts for pay-in-full, offer cash pay, and provide cash services such as dry needling as well!
